چکیده

A 69-year-old patient with unstable angina presented to cardiac catheterization with a tight lesion in the mid right coronary artery (Figure 1A) and moderate lesions in the left main and left anterior descending coronary arteries. During coronary angioplasty, a stent was electively implanted in the right coronary artery, with good initial results. However, during subsequent aggressive, high-pressure dilations performed to “optimize” stent expansion (using both angiographic and intravascular ultrasound criteria), a vessel rupture was noticed. The angiographic image revealed a faint but progressive smoke-like extravasation of contrast within the stented segment (Figure 1B, curved arrow). Protamine administration, prolonged balloon inflations, and further proximal stent implantation (partially overlapping the previous stent) were unsuccessful in closing the site of blood leakage into the pericardial space (Figure 1C). Intravascular ultrasound visualized, from “within the artery,” the stent wall apposed to the vessel wall and the site of vessel rupture encompassed by 2 struts of the stent (Figure 2A, arrow). Hand injection of saline opacified the vessel lumen and also accurately located the site of contrast exit outside the vessel (Figure 2B). Eventually, cardiac tamponade ensued, and a pericardial tap was necessary to regain hemodynamic stability. At surgery, a red clot was demonstrated overlying the mid right coronary artery. When this thrombus was aspirated, a 232-mm hole in the vessel wall permitted partial visualization of 1 metallic filament of the stent. Coronary artery bypass grafting of the right and left coronary arteries was performed successfully, and the patient had an uneventful postoperative course.

A Case of Balloon Rupture Cause Serious Complication during Percutaneous Coronary Intervention

Balloon rupture in the process of percutaneous coronary intervention (PCI) leading to coronary artery dissection, subintimal hematoma and hematoma extension after stent implantation is an uncommon complication, but it has important clinical signifi cance. The reason for balloon rupture during PCI and hematoma extension after stent implantation is not clear. In this case, Balloon rupture may be ...

Modeling of stent implantation in a human stenotic artery 0850v1.docx Modeling of stent implantation in a human stenotic artery

The aim of this work is to introduce a methodology to study the stent expansion and the subsequent deformation of the arterial wall towards the outside direction in order arterial lesion to be rehabilitated and blood flow to be restored. More specifically, a coronary artery and the plaque are reconstructed using intravascular ultrasound and biplane angiography. The finite element method is used...

Percutaneous valved stent implantation above the coronary ostia.

BACKGROUND In recent years, some experimental and clinical studies on transcatheter aortic valve implantation (TAVI) have been conducted. TAVI is indicated in patients with calcified pure or predominant aortic stenosis. The risk of this technique is still high. Aortic valved stent implantation above the coronary ostia might avoid blocking the coronary ostia. METHODS AND RESULTS Twenty healthy...

Rupture and Spontaneous Sealing of a Coronary Aneurysm After Deployment of Drug-Eluting Stent

Lesions with coronary artery aneurysm (CAA) can become complicated during percutaneous coronary intervention. Here, we report a case of a 78-year-old man who developed a rupture, and spontaneous sealing of the CAA occurred after stent implantation, as shown by computed tomography coronary angiography.

Coronary stenting and inflammation Stent coronário e inflamação

coronary artery disease has dramatically altered the practice of coronary artery bypass grafting surgery. New complications caused by percutaneous coronary interventions (PCI) with the implantation of stents has emerged and affected the result when surgery is subsequently required. The stent was initially utilized for the treatment of complications associated with percutaneous coronary angiopla...
